Schedule: 28 to 32 hours per week (Depending on experience and availability)

Job Description: The Community Health Worker (CHW) is a trained home visitor and an integral member of a health team. CHWs connect underserved clients to primary care, health insurance, assist in navigation of resources, and support clients by addressing their overall health and social needs under the supervision of a qualified health professional.
