Location: Middletown, NY
Pay Rate: $21.63 per hour
Hours: Monday-Friday 8:00AM-4:30PM (40hrs)
OVERVIEW OF PRIMARY RESPONSIBILITES : The Interviewer organizes activities related to the scheduling and completion of Personal Outcome Measures interviews. PRIMARY FUNCTIONS : Completes up to 20 POMs interviews with associated reports each month.
Ensures POMs interview reports are entered into our EHR system.
ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT: Functions as one of the subject matter experts (SME) to help facilitate questions around Personal Outcome Measures.
QUALIFICATIONS AND ATTRIBUTES : Ability to perform effectively both independently and as a team member.
Proficient use of technology (Microsoft Office 365 apps/services - Outlook, Word, Excel, etc.).
Proficient in use of electronic health records (such as Evero).
Proficient use of virtual platforms such as Zoom and Microsoft Teams.
Excellent organizational skills.
Ability to engage in travel to various sites to perform Personal Outcome Measures interviews
Demonstrated success in working with diverse teams.
Exceptional written and oral presentation skills to effectively communicate new ideas, concepts, and practices.
The ideal candidate will have the flexibility to work some non-traditional hours which would include evenings, and weekends.
EDUCATION AND EXPERIENCE : Bachelor’s Degree preferred but not required
Experience providing services to adults with intellectual and developmental disabilities.
Current Personal Outcome Measures interviewer certification preferred or able to be trained upon hire.
